Registration Forms
If you have already scheduled an appointment, please check your email for your registration forms. These forms will be sent to you before your appointment, but depending on how far out your appointment is scheduled, may not arrive immediately after you have booked your appointment.
If you have not received your paperwork at least two days before your appointment, please check your spam folder. If you still cannot find the paperwork, please don't hesitate to call us at (508) 650-0060.
The email will come from FlexPlus Physical Therapy and the subject line will read "Intake Form (date sent)".
The body of the email will look like this:
Click the green "Fill Out Patient Intake" and enter your date of birth to begin filling out the forms.
If you have any questions regarding these forms, please do not hesitate to reach out. We are happy to help in any way that we can. Alternately, feel free to arrive in our office twenty minutes before your appointment time to fill these pages out.
